The Health Visiting Service is made up of six teams (A-F) divided into six geographical areas across both boroughs. The teams are aligned to local Children Centres (in Hackney and the City) and GP surgeries (in Hackney and the City).

Serving GP practices at Allerton Road Medical Centre, Barretts Grove Surgery, Cedar Practice, Heron Practice, Barton House Surgery and Cranwich Road Surgery.
John Scott Health Centre
Team B
Serving the Stamford Hill Practice, Clapton Surgery, Dr Gadhvi Practice, Tollgate Lodge PC Centre, Healy Practice, Elm Practice, Rosewood Practice, Riverside Surgery and Spitzer & Partners.
Fountayne Rd Health Centre
Team C
Serving the Somerford Grove practice, Dr Prasad’s Surgery, the Sandringham Practice, the Somerford Grove practice, Dr Prasad’s Surgery, the Dalston Practice and the Nightingale Practice.
Hackney Ark

Serving GP practices at the Athena Medical centre, the Kingsmead Surgery, the Trowbridge Surgery, the Sorsby Health Centre, the Lea Surgery, the Wick Health Centre and the Lower Clapton Health Centre.

Serving the Lawson Practice, the De Beauvoir Practice, the City Road Medical Centre, Pitfield Surgery, Strouts Place Medical Centre/Defoe Geographical, Whiston Road Practice, Southgate Road MC, Queensbridge Group, Beechwood MC, St. Peters Medical Practice.
